​Prescription Upload Instructions​

  1. Upload your prescription here.
  2. Make sure the photo is clear and complete — all corners visible, no blur, no glare, and nothing cropped.
  3. Check that all parts are captured in the image, including:
    • Patient name
    • Doctor’s name & signature
    • Clinic details/contact info
    • Prescription date
    • Medicine name, dosage, and instructions
  4. After you upload, our pharmacist will call you to verify the prescription details before we process your order.

Tip: If the prescription is long, you may upload multiple photos (top to bottom) so everything is readable.

By checking the box, I understand and agree that by uploading my prescription to Shield Drugstore, Shield Drugstore may collect, view, store, and use the information I provide only to:

  • verify and validate the prescription details,
  • contact me through the pharmacist or authorized staff for clarification/confirmation,
  • prepare and dispense the requested medicines (and arrange delivery/pickup if selected), and
  • comply with applicable Philippine laws and regulatory requirements.

I understand that Shield Drugstore may share only the necessary details with its delivery partner/courier solely for delivery, limited to my name, contact number, and delivery address (if delivery is requested).

Shield Drugstore will restrict access to my information to authorized personnel only and will not sell or use my prescription or personal data for marketing without my consent.

I confirm that the prescription I upload is mine, or I am authorized to upload it on behalf of the patient, and that the information I provided is accurate.
